Angel Ocasio is a FEMA applicant services representative working at the State/FEMA Disaster Recovery Center at the Little Axe Elementary School. His job is to ensure that survivors get everything they are eligible to receive. Sometimes that means helping the survivor figure out what documents they may need to keep their disaster assistance applications moving along.
Many documents can be replaced by contacting the companies or agencies which issued them. Some of the places you may want to contact include:
State of Oklahoma
The state of Oklahoma offers multiple resources for replacing lost documents for survivors of the May tornadoes:
- Supplemental Nutrition Assistance Program card replacements –Oklahoma Department of Human Services
- Driver’s licenses, IDs and proof of ownership – Oklahoma Tag
- Birth and death certificates – Oklahoma State Health Department (Fees will be waived through June 30, 2013)
- Free replacement diplomas for OU graduates – University of Oklahoma
Federal Agencies
The federal government provides replacement documents for survivors of federally-declared disasters:
