MT-2 Application Forms and Instructions

Application Forms for Conditional Letters of Map Revision (CLOMR) and Letters of Map Revision (LOMRs) (For all LOMR, CLOMR, and Physical Map Requests)

The application forms and instructions included in the MT-2 forms package were designed to assist requesters (community officials or individuals via community officials) in gathering the data that the Federal Emergency Management Agency (FEMA) needs to determine whether the effective National Flood Insurance Program (NFIP) map (i.e., Flood Hazard Boundary Map, Flood Insurance Rate Map, Flood Boundary and Floodway Map, or Digital Flood Insurance Rate Map) and Flood Insurance Study report for a community should be revised. These forms, which expire on December 31, 2010, also should be used by community officials or individuals via community officials for requesting FEMA comments on a proposed project issued in the form of a CLOMR. These forms assure FEMA that all pertinent data relating to the request is included in the submittal. They also ensure that:

Endangered Species Act (ESA) Requirements

For Letter of Map Change Requests (LOMCs) involving floodplain activities that have already occurred, private individuals and local and state jurisdictions are required to comply with the ESA requirements independent of FEMA’s review. These as-built requests do not provide the same opportunity as Conditional LOMCs for FEMA to comment on the project because map changes are issued only after the physical action has occurred. Confirmation Receipt and Payment Form

For confirmation of receipt and assigned case number, please include an e-mail address in the completed MT-2 application package, Form-1, entitled Overview & Concurrence Form.

The FEMA Payment Information Form must accompany the application forms and technical data submitted in support of CLOMR and LOMR requests. The form also is to be used by individuals submitting requests for technical and administrative support data from the FEMA library archive.

Where to Send Requests, Including Review and Processing Fees, for CLOMRs and LOMRs

As outlined in the instructions for completing the application forms for CLOMR and LOMR requests, please continue to submit both the review and processing fee and the application package together to the address shown on page 4 of the "Instructions for Completing the Application Forms for CLOMR and LOMR Requests."
