MT-2 Application Forms

Application Forms for Conditional Letters of Map Revision (CLOMR) and Letters of Map Revision (LOMRs) (For all LOMR, CLOMR, and Physical Map Requests)

The application forms included in these files were designed to assist requesters (community officials, individual property owners, and others) in gathering the information that the Federal Emergency Management Agency (FEMA) needs to determine whether property (i.e., parcel[s] of land, structure[s]) is likely to be flooded during the flood event that has a 1-percent chance of being equaled or exceeded in any given year. The 1-percent-annual-chance flood is often referred to as the base, or 100-year flood.

The forms in this package are to be used to request CLOMRs and LOMRs.

Confirmation Receipt and Payment Form

For confirmation of receipt and assigned case number, please include an e-mail address in the completed MT-2 application package, Form-1, entitled Overview & Concurrence Form.

The FEMA Payment Information Form, which is also to be used by individuals submitting requests for technical and administrative support data from the FEMA library archive, must accompany submittals for CLOMR and LOMR requests.

Where to Send Conditional and Final Map Revision Requests, Including Review and Processing Fees

As outlined in the instructions for completing the application forms for CLOMR and LOMR requests, please continue to submit both the review and processing fee and the application package together to the address shown in page 4 of the "Instructions for Completing the Application Forms for CLOMR and LOMR Requests."

All requests for CLOMRs and LOMRs for locations within the most of the country except for areas within State of North Carolina, and Denver Metropolitan Area (Urban Drainage and Flood Control District) should be mailed to:
