Mitigated Properties Updates

In order to maintain accurate, up-to-date records of all repetitive loss properties mitigated as a result of FEMA mitigation grants, FEMA requires the submission of Form AW-501, National Flood Insurance Program (NFIP) Repetitive Loss Update Worksheet (OMB #1660-0022).  Typically, this form is completed by the State or community, but may be completed by a Region with appropriate documentation that shows any changes in the status of a property (e.g., elevation certificate).  This form, along with the transmittal sheet or other document signed by an authorized community official, must be submitted for each property mitigated with Flood Mitigation Assistance (FMA) funds prior to closeout.  Form AW-501 and instructions for completing and submitting the Form are available via the links below:
