What should I expect when completing the questionnaire?
The questionnaire should take only a few minutes to complete. You are not required to answer all questions but you will get the best results if you do. Most of the questions are specifically related to disasters. Others will help determine your eligibility for different types of help. This is an anonymous questionnaire so your answers are not saved and will not be used by any Federal agency.
After you finish, you will receive a list of assistance that you may be eligible for based on your answers to the questions.
