NIMS Incident Command System (ICS) Forms

The National Integration Center developed NIMS Incident Command System (ICS) Forms for use by all incident command organizations. The forms reflect extensive stakeholder input and pilot testing over the last 18 months. The forms are all-hazard and establish the required fields for each form. 

The National Integration Center recognizes that organizations may want to apply these forms for specific hazards or disciplines. Therefore, each form contains specific instructions to ensure consistent use nationally as well as instruction for organizations to extend them for specific circumstances.
