This document is a sample disclosure agreement between FEMA and a model developer. The agreement is enacted prior to a model being added to the list of acceptable models.

This document is an agreement between the model developer and an appellant, which provides for the release of the source codes and user's manuals to the appellant, and ensures that the appellant will protect the proprietary rights of the model developer.
