The application forms and instructions included in the MT-1 package were designed to assist requesters (community officials, individual property owners, and others) in gathering the information that the Federal Emergency Management Agency (FEMA) needs to determine whether property (i.e., parcel[s] of land, structure[s]) is likely to be flooded during the flood event that has a 1-percent chance of being equaled or exceeded in any given year. The 1-percent-annual-chance flood is often referred to as the base, or 100-year flood. The area flooded by the 1-percent-annual-chance flood is called the Special Flood Hazard Area (SFHA). The mandatory flood insurance purchase requirements of the National Flood Insurance Program (NFIP) apply to insurable structures in the SFHA shown on an effective NFIP map.

Endangered Species Act (ESA) Requirements
For Letter of Map Change Requests (LOMCs) involving floodplain activities that have already occurred, private individuals and local and state jurisdictions are required to comply with the ESA requirements independent of FEMA’s review. These as-built requests do not provide the same opportunity as Conditional LOMCs for FEMA to comment on the project because map changes are issued only after the physical action has occurred. The FEMA Payment Information Form is to accompany requests for CLOMA, LOMR-F, and CLOMR-F requests when a review and processing fee is required. This form also is to be used by individuals submitting requests for technical and administrative support data from the FEMA library archive.
