The application forms and instructions included in the MT-1 package were designed to assist requesters (community officials, individual property owners, and others) in gathering the information that the Federal Emergency Management Agency (FEMA) needs to determine whether property (i.e., parcel[s] of land, structure[s]) is likely to be flooded during the flood event that has a 1-percent chance of being equaled or exceeded in any given year. The 1-percent-annual-chance flood is often referred to as the base, or 100-year flood. The area flooded by the 1-percent-annual-chance flood is called the Special Flood Hazard Area (SFHA). The mandatory flood insurance purchase requirements of the National Flood Insurance Program (NFIP) apply to insurable structures in the SFHA shown on an effective NFIP map.
applicants Please Note: Expired MT-EZ, MT-1, and MT-2 forms may be used until further notice. These forms will continue to be accepted when submitting for a Letter of Map Change (LOMC).

The forms in this package are to be used to request multiple-lot/multiple-structure LOMAs, CLOMAs, LOMR-Fs, and CLOMR-Fs, in paper copies, via mail. Online LOMC is also available for submitting applications and payments (if required) online.
Endangered Species Act (ESA) Requirements
For Letter of Map Change Requests (LOMCs) involving floodplain activities that have already occurred, private individuals and local and state jurisdictions are required to comply with the ESA requirements independent of FEMA’s review. These as-built requests do not provide the same opportunity as Conditional LOMCs for FEMA to comment on the project because map changes are issued only after the physical action has occurred. You can view a table that provides a general summary of FEMA's ESA requirements.
