These forms expire February 28, 2014, and should be used to request that FEMA remove structures or legally recorded parcels of land or portions thereof, described by metes and bounds certified by a Registered Professional Engineer or a Licensed Land Surveyor, from a designated Special Flood Hazard Area (SFHA) via Letter of Map Amendment (LOMA) or Letter of Map Revision Based on Fill (LOMR-F). These forms are also used for requesting FEMA’s comments on a proposed project issued in the form of a Conditional Letter of Map Amendment (CLOMA) or Conditional Letter of Map Revision Based on Fill (CLOMR-F). These forms assure FEMA that all pertinent data relating to the request is included in the submittal.
